Required Digital Forms:
All digital forms are located in your onCampus account. Available/unsigned medical, tech, and permission forms can be accessed through a link in a yellow banner at the top of the page.

Medical Information Update
All students must have a medical form submitted before they can attend school. Medical information from the previous year will roll to this year's form. Please take time to review the information and add any new or additional information as needed before submitting. This form is due by Registration/Orientation Day.

Technology Agreement Form
All students in grades SrK - 12 will have access to technology provided by the IT Department. A digital tech agreement form is available for you to sign in onCampus . All policies can be found on the student laptop page.
NOTE: All 5th grade students will need to attend a laptop pickup meeting with a parent. If you are have a new 6th - 12th grade student who is unable to attend a New Student Summer Session, you will need to attend a laptop pickup meeting. See the Student Laptop page for information, dates, and times.

Grade Specific Event Forms (6th - 12th Grade)
Parents with students in grades 6 - 12 will find a BCS Events Form for dances as well as Student Retreat Forms. 12th grade parents will also find a Project Graduation form. All forms must be signed by one parent/guardian and some also require a signature by the student.

Parent Parking Decal
All parents must have a new parent parking sticker on their vehicles each school year. Login to your onCampus parent account and go to the "Resource" board. Click on the link for parent parking decal. Once completed and submitted online, your new decal will be mailed to you. Each family receives two free decals each year. After that, the cost is $5 each and can be paid online with the online form. This form is available starting in early-July.

Emergency Contacts
Each student must have parent information listed in their Emergency Contacts by the first day of school. If you have a grandparent or babysitter who will be picking your child up from school at any time during the school year, please list them as an emergency contact.
How to Manage Emergency Contacts
A parent will need to pick up his/her child within 30 minutes of receiving a call from the school. Please list both parents as the emergency contact, if possible, in the order you wish to be called. Please list another emergency contact who can pick up your student other than yourself in case of emergency.

Parent Profile Information
Correct parent contact information (such as phone numbers, home address, and parent email addresses) is extremely important to our teachers, administrators and office employees. To update any of your parent contact information, please log into your onCampus account and find the Contact Info Update Form on the Resource Board.
